We are currently seeking experienced Service Coordinators to support care management operations across multiple regions in Indiana.
Position Summary
As a Service Coordinator, you will serve as a primary point of contact for a panel of older adult members with complex medical, behavioral health, and social needs. You will collaborate with members, providers, and community resources to improve health outcomes, increase access to care, and address social determinants of health.
This position offers a hybrid work environment, combining community-based member visits, telephonic outreach, and remote work.
Work Schedule: Monday – Friday, 8:00 AM – 5:00 PM
Travel Requirements: Up to 75% local travel within assigned territory. Mileage reimbursement is provided at the current federal reimbursement rate.
Primary Responsibilities
- Serve as the primary care manager for members with complex medical, behavioral health, and social needs.
- Conduct comprehensive assessments through face-to-face and telephonic engagement, including evaluation of medical, behavioral, functional, cultural, and socioeconomic needs.
- Develop and implement individualized, person-centered care plans that support members' health goals and readiness for change.
- Coordinate services and collaborate with interdisciplinary care teams, healthcare providers, and community organizations.
- Connect members with appropriate resources and support services, including housing assistance, transportation resources, food security programs, pharmacy support, behavioral health services, and other community-based programs.
- Monitor member progress and adjust care plans as needed to support positive health outcomes and quality of life.
- Maintain accurate and timely documentation within required systems and platforms.
- Advocate for members and assist in overcoming barriers to care and service access.
Required Qualifications
Candidates must reside in Indiana and possess one of the following qualifications:
Education and Licensure
- Current, unrestricted Registered Nurse (RN) license in Indiana; OR
- Master's Degree in Social Work; OR
- Bachelor's Degree in Social Work, Psychology, Special Education, Counseling, Human Services, or a related field, with at least one (1) year of case management experience supporting older adults, individuals with disabilities, or individuals with serious mental illness.
Experience
- Minimum one (1) year of experience working with behavioral health providers, community-based organizations, or social service agencies addressing Social Determinants of Health (SDoH).
- Minimum one (1) year of experience using Microsoft Office applications, including Word, Excel, and Outlook.
- Minimum one (1) year of experience working with older adults, individuals with disabilities, or long-term support services.
- Ability to travel up to 75% within the assigned service area.
- Reliable transportation and a valid driver's license.
Preferred Qualifications
- Training or certifications in Motivational Interviewing, Stages of Change, Trauma-Informed Care, or Person-Centered Care.
- Experience working within interdisciplinary or team-based care environments.
- Experience supporting individuals with co-occurring mental health and substance use disorders.
- Managed care experience.
- Bilingual skills, particularly Spanish or languages commonly spoken within assigned service areas.
Physical Requirements
- Ability to transition between remote work, office settings, provider locations, and member homes throughout the workday.
- Ability to navigate a variety of community and residential environments.
- Ability to remain seated and work on a computer or tablet for extended periods when completing documentation and care coordination activities.
