About the Role
osition Overview:
The Obstetrics Case Manager RN is responsible for developing, assessing, and facilitating complex care management activities, primarily for physical health needs. This includes creating personalized care plans, providing education to members and their families, and collaborating with healthcare providers to address and meet the needs of members. The ideal candidate will have experience in an obstetrics setting, such as labor and delivery.
Key Responsibilities:
Develop, assess, and facilitate care management activities, focusing on physical needs to provide high-quality, cost-effective healthcare outcomes.
Create personalized care plans and provide education to members and their families regarding disease processes, healthcare instructions, and available care options.
Collaborate with providers to identify specialists, community resources, and services to meet the member’s needs.
Identify barriers to care and facilitate appropriate care management interventions to address challenges.
Coordinate communication between members, family/caregivers, and the care provider team to ensure access to person-centered care and services.
Conduct ongoing follow-up and monitoring of member status, progress towards care plan goals, and revise plans as necessary to meet the member's evolving needs.
Provide resource support to members and care managers, connecting them with local resources (e.g., housing, employment, healthcare, independent living) based on assessments and care plans.
Facilitate access to timely and appropriate care through collaboration with providers and specialists.
Perform telephonic, digital, or home outreach to assess member needs and collaborate with resources.
Maintain and document all member information and care management activities in compliance with state, federal, and third-party payer regulations.
Provide feedback to leadership on opportunities for improving care delivery and quality in a cost-effective manner.
Requirements
Education and Experience Requirements:
Bachelor’s degree in Nursing or a Degree from an Accredited School of Nursing.
2-4 years of related nursing experience, ideally in an obstetrics or labor and delivery setting.
License/Certification Requirements:
RN – Registered Nurse with State Licensure and/or Compact State Licensure.
Skills and Qualifications:
Strong clinical knowledge, with experience in case management, obstetrics, or healthcare settings.
Excellent communication skills to collaborate with members, families, and healthcare providers.
Ability to assess, develop, and manage care plans tailored to the unique needs of each member.
Knowledge of social determinants of health and ability to address barriers to care.
Ability to provide resource support and referrals to ensure comprehensive care management.
Proficient in maintaining accurate records and adhering to compliance regulations.
