About the Role


Position Overview:

We are seeking a dynamic and highly organized Population Health Leader. This is a new position designed to lead and coordinate efforts across our Population Health initiatives. The ideal candidate will bring creative problem-solving with experience in health plans, data analysis, quality improvement, and population health management. In this role, you will work closely with team members to drive solution-oriented discussions, foster productive collaboration, and lead cross-functional teams to achieve key health outcomes.

As the Population Health Leader, you will take the lead in organizing and managing workstreams, analyzing data, and providing actionable insights to improve the quality of care for our members.

You will work in a hybrid setting, balancing time between in-office and remote work. The role requires strong leadership skills, excellent written and verbal communication, and a creative, flexible approach to problem-solving as we refine and optimize our processes.

Key Responsibilities:
Lead Population Health Workstreams: Organize, manage, and lead cross-functional teams to analyze data, solve complex challenges, and implement solutions that improve population health outcomes
Data Analysis and Reporting: Utilize data analysis skills to assess population health trends, identify gaps in care, and recommend strategic interventions. Translate complex data into actionable insights for the team and stakeholders.
Quality Improvement Initiatives: Lead and/or contribute to quality improvement projects, ensuring that population health interventions are aligned with best practices and regulatory standards.
Project Management: Apply project management methodologies to oversee timelines, resources, and deliverables for population health initiatives. Ensure that team goals are met on time and within scope.
Goal Setting and Team Leadership: Set clear, measurable goals for teams and individuals. Foster a collaborative and high-performance environment while ensuring that goals are aligned with broader organizational objectives.
Continuous Improvement: Identify areas for process improvement, contribute creative solutions, and lead efforts to refine workflows and practices. Adapt to evolving needs and challenges as the team matures in its operations.
Communication and Reporting: Prepare written reports, presentations, and other communication materials to update leadership on progress, challenges, and outcomes related to population health initiatives.

Requirements

Qualifications:

  • Experience:

    • Experience in a healthcare-related field, with a focus on population health, health plans (including Medicaid), and/or quality improvement.

    • Experience analyzing healthcare data to identify trends, opportunities for improvement, and actionable next steps.

    • Strong understanding of quality improvement methodologies and project management principles.

  • Skills:

    • Excellent leadership, organizational, and communication skills (both written and verbal).

    • Ability to convene and facilitate discussions among cross-functional teams to solve complex problems.

    • Proficiency in data analysis, with experience using tools like Excel, Power BI, or similar platforms.

    • Ability to lead teams in a remote or hybrid work environment while maintaining engagement and productivity.

    • Creative problem-solving abilities, with a flexible and adaptable mindset to navigate a new and evolving role.

  • Preferred:

    • Familiarity with Medicaid and managed care environments.

    • Experience with quality improvement frameworks (e.g., PDSA, Lean, Six Sigma).

    • Project management certification (PMP or similar) is a plus.
